Critical sections must be demonstrated competently for an unrestricted pass. This form does not replace equipment-specific instructions, risk assessments or a suitable qualified instructor.

Complete before a loaded tail-lift demonstration. Figures must be verified rather than guessed. Do not proceed if compatibility or loading limits cannot be confirmed.
